We request that return shipments be authorized by IQ Dental Supply, and please call one of our customer service representatives at 1-888-303-1115 and request an authorization number before returning your shipment.
To receive full merchandise credit, items must be in good condition (unused), in the original packaging and returned within 30 days of the invoice date. To expedite your credit, please include a copy of your original invoice and a note explaining the reason for return. Opened handpieces and equipment may not be returned for credit, but will be repaired or replaced in accordance with the manufacturers' warranty. Special Order items are not returnable.
All orders throughout the Continental U.S will be subject to a handling charge of $8.95. This charge includes fuel surcharges and handling, except for additional carrier charges related to hazardous material shipments. Shortages or errors in shipment must be reported within 7 days of the invoice date to issue credit (when applicable).
